You can’t go back to safety-sensitive work until you complete the return-to-duty process with a qualified DOT Substance Abuse Professional (DOT SAP). We’ll walk you through every step clearly, professionally, and without judgment.

Evaluations are available in person at our Baltimore office (1340 Smith Avenue, Suite 200) or by secure live video for employees anywhere in Maryland, as permitted under 49 CFR 40.291. One flat fee of $550 covers both your initial and follow-up evaluations.

Evaluations are conducted by Charmaine Nalty, LCSW-C, a DOT-qualified Substance Abuse Professional under 49 CFR Part 40.

CDL drivers: If your Clearinghouse status is “prohibited,” Maryland will downgrade your CDL until you complete the return-to-duty process, so it pays to start early.

Who Needs a DOT SAP Evaluation?

Employees in safety-sensitive positions covered by DOT drug and alcohol testing rules, including:

• FMCSA: CDL truck and bus drivers
• FAA: pilots, flight attendants, aircraft mechanics, and air traffic controllers
• FTA: transit vehicle operators and mechanics
• FRA: railroad engineers, conductors, and other covered workers
• PHMSA: pipeline workers
• USCG: commercial vessel crew members

The DOT Return-to-Duty Process

1. Initial evaluation. A clinical assessment of your substance use history and the violation, in person in Baltimore or by live video anywhere in Maryland.
2. Recommendation. We recommend education and/or treatment based on the evaluation. Every employee who has a violation receives at least an education recommendation.
3. Complete your plan. You complete the recommended education or treatment with an approved provider.
4. Follow-up evaluation. We confirm whether you’ve successfully complied with the recommendation.
5. Report to your employer. We send our report to your Designated Employer Representative (DER), along with a follow-up testing plan.
6. Return-to-duty test. You take a return-to-duty test, which must come back negative. Your employer decides whether you return to work.

CDL drivers: Before your first appointment, you must designate us as your DOT SAP in the FMCSA Drug & Alcohol Clearinghouse. You’ll need to log in to your Clearinghouse account and designate Charmaine Nalty as your DOT SAP.

Video evaluations follow DOT rules: they must use real-time audio and video that lets the evaluation be just as thorough as an in-person visit. You must be physically located in Maryland at the time of your video evaluation.

DOT SAP Evaluation FAQs

How much does a DOT SAP evaluation cost?

$550. This covers both your initial evaluation and your follow-up evaluation. The cost of any education or treatment program you are referred to is separate.

Can I do my DOT SAP evaluation online?

Yes, if you are in Maryland. DOT rules (49 CFR 40.291) allow DOT SAP evaluations by live video with real-time audio and visual interaction. We also offer in-person evaluations at our Baltimore office.

How long does the return-to-duty process take?

It depends on the education or treatment recommended in your initial evaluation. Once you complete it, you return for a follow-up evaluation. If you have successfully complied, we report that you are eligible for a return-to-duty test, which your employer arranges.

I’m a CDL driver. What do I need to do in the FMCSA Clearinghouse?

You will need to designate your DOT SAP in the FMCSA Drug & Alcohol Clearinghouse. Once you designate Charmaine Nalty as your DOT SAP, she can accept and record your evaluation dates and return-to-duty eligibility there.

Do I still need a DOT SAP if I quit or was fired?

Yes. A DOT violation stays with you. You cannot perform safety-sensitive work for any DOT-regulated employer until you complete the return-to-duty process with a qualified DOT SAP.

Which DOT agencies do you work with?

We evaluate employees covered by FMCSA (CDL drivers), FAA, FTA, FRA, PHMSA, and USCG drug and alcohol testing rules. We also provide NRC Substance Abuse Expert (SAE) evaluations for nuclear workers under 10 CFR Part 26.
